Practical support, clearly planned.

During advanced illness, families often need both practical support and a calm, compassionate presence. TCC can work alongside the client’s healthcare team and family to support comfort, routines and quality of life at home.

  • Personal comfort, hygiene, and positioning support according to the care plan.
  • Companionship, reassurance, and quiet presence for the client and family.
  • Meal, hydration, and household assistance as appropriate to the client’s needs.
  • Respite support so family caregivers can rest or attend to other responsibilities.
  • Licensed nursing care when clinical tasks are required and appropriately ordered.
  • Coordination and communication with family and other care providers based on consent and the agreed plan.
