Coming home after hospitalization can mean new routines, mobility changes, follow-up appointments, and rehabilitation instructions. TCC can provide practical home-care support and, when clinically appropriate, licensed nursing services that complement the discharge and rehabilitation plan.
- Help with mobility, transfers, and everyday routines according to the discharge or care plan.
- Personal support, meals, hydration, and a comfortable transition back to home routines.
- Support with clinician- or therapist-directed rehabilitation routines when they are documented in the care plan and within staff scope.
- Medication reminders and organization support according to the established medication plan.
- Transportation or accompaniment for follow-up appointments when arranged.
- Licensed nursing support for ordered clinical tasks, including wound-care needs, when clinically appropriate and within scope.
